Non-surgical management of endometrial cancer: ESGO–ESTRO–ESP 2025 guideline

Endometrial cancer (EC) is a common gynecological malignancy with a rising incidence. The molecular classes described by the The Cancer Genome Atlas (TCGA) Network more than a decade ago have opened new perspectives in the postoperative management and palliative treatment of EC. In this context, the International Federation of Gynecology and Obstetrics updated the staging system for EC in 2023, while the joint guidelines of the European Society of Gynaecological Oncology, the European Society for Radiotherapy and Oncology, and the European Society of Pathology have been revised on three occasions since 2016. The continuously expanding body of pathological and molecular knowledge, along with the associated therapeutic options, is comprehensively summarized in the ESGO–ESTRO–ESP guideline published in 2025. In this paper, we aimed to provide an overview of the non-surgical aspects of these recommendations.
