Pathology of endometrial cancer

In the last decade the most advancement in the field of gynecological pathology was made in the topic of endometrial carcinomas. New pathological entities were described such as mesonephric-like endometrial carcinoma and gastric-type mucinous endometrial carcinoma. Molecular classification was also introduced and had an effect on treatment algorithms and on staging as well. Now there are several pathological factors needed to determine the adequate clinical stage such as histological type, grade, pathological stage and lymphovascular invasion. Along with these the role of pathologists and the conversation between pathologists and oncologists became very important. Certain immunohistochemical examinations are needed for the molecular classification but also for the correct histotyping. Immunohistochemical evaluation of p53, mismatch repair and hormone receptor status is obligatory in every endometrial carcinoma. It is especially im portant to distinguish low-grade and high-grade endometrial carcinomas. For the reasons above the reporting of endometrial carcinomas is ideally done by an expert gynecological pathologist.
